  • Information for Leasing a Specialist Disability Accommodation (SDA) Property
    Property Viewings
    When attending a viewing, we encourage you to bring members of your support network, such as:
    Family members or guardians
    Occupational Therapist (OT)
    Supported Independent Living (SIL) provider
    Support Coordinator (SC)
    Any other professionals or people who assist you in decision-making
    This ensures the property is suitable for your individual support needs and living requirements.
     
    Application Process
    If the property meets your needs, you will be asked to complete a SDA Participant Application Form.
    This form allows us to collect the required personal information to prepare the leasing documentation. During your application you will be required to provide evidence of your SDA funding.
    Your care provider will also need to complete a collaboration application.

    Tenant Costs
    As a tenant, you will be responsible for the following costs:
    Reasonable Rent Contribution (RRC)
    $258 per week – This value is set by the NDIS and while it increases twice per year we will only increase it once per year.
    This amount is generally paid from your Disability Support Pension (DSP) and Commonwealth Rent Assistance (CRA). You can apply and receive the fully amount of CRA if you are paying reasonable rent contribution.
    Security Bond
    4 weeks of RRC plus 2 weeks rent in advance
    Total payable prior to moving in: $1,532
    Utilities and Services
    Tenants are responsible for arranging and paying for:
    Electricity
    Gas (if applicable)
    Internet
    These services can be organised either by you directly or through your SIL provider.
     
    Board
    In a shared house a SIL provider is expected to arrange the utilities and charge you board. If you are paying directly for utilities the SIL provider cannot charge you a board payment unless they itemise the specific charges. Board must include meals and consumables, utilities, access to white goods and laundry and furniture and furnishing in the common areas.
    The maximum board payment a SIL provider can charge is 50% of the disability support pension and 100% of the energy supplement.
     
    Water Usage
    GR8 Disability Housing will provide invoices for water usage during your tenancy. These can be paid by yourself or your SIL provider depending on your board arrangements.
     
    Documents Required Before Moving In
    Before moving into the property, the following documents must be reviewed and signed:
    ·      Residential Tenancies Act Lease Agreement
    This agreement outlines your rights and responsibilities as a tenant, including your commitment to pay the weekly Reasonable Rent Contribution (RRC).
    ·      GR8 SDA Service Agreement
    This document outlines the roles and responsibilities of you and GR8 and confirms that GR8 Disability Housing is authorised to claim SDA funding from your NDIS plan.
    ·      NDIS Consent to Share Information
    This form allows GR8 Disability Housing to contact the NDIS in relation to any SDA funding issues.
     
    Actions Required Before Moving In
    There are a couple of actions you need to undertake to notify the NDIS of your move into SDA.
    ·      Submit Change of Situation Form to the NDIS
    Prior to moving in GR8 will provide an SDA quote for you for the property. The SDA quote needs to be submitted to the NDIS along with Change of Situation Form advising the NDIS of your move into an SDA property, your new address, your move in date and request the correct SDA funding be added to your plan.
    ·      Add GR8 Disability Housing as your SDA provider in the my NDIS Participant Portal
    You will need to notify the NDIS that GR8 Disability Housing is your SDA provider on PACE. This can be done through a relationship request through the NDIS portal or by calling the NDIS.
     
    Property Maintenance and Damage Responsibilities
    Tenants are responsible for maintaining the property and keeping it damage free including the garden and outdoor areas of the property. This includes ensuring the reticulation (watering system) is operating correctly and set to run twice per week expect during winter.
    If you damage the property, you are responsible for paying for the repair or replacement of the damage.
    If something breaks down in the property you need to notify us so that we can arrange for it to be repaired.
     
    Furnishings and Property Modifications
    All properties are provided unfurnished and are leased as viewed during the property inspection.
    The property will only include the modifications that are present at the time of viewing.
    If additional modifications are required to support your individual needs:
    A request must be submitted in writing to pm@gr8corp.com.au using the Minor or Major Modifications Request Form.
    The request will be forwarded to the property owner for approval.
    If approved, the modifications must be funded by yourself or through your NDIS plan, typically under Assistive Technology funding.

  • This is a multipage application form. To complete this application, you will require the following documentation:

    • A copy of the participants identification such as a birth certificate, passport, drivers' license, proof of age card or other photo ID
    • A copy of the participants NDIS Plan or, at a minimum the Home Modifications section of the NDIS plan

  • Processing Your Application

  • Your application will be processed with the information provided and submitted. If you are offered a vacancy and accept, an initial assessment will be undertaken to ensure your preferences and needs are documented and taken into account.

    An SDA Service Agreement will then be prepared and will require acceptance and signature from all parties before your SDA residency commences.

    Where a decision regarding whether a vacancy cannot be offered, the Property Manager will contact you or your representative within 2 working days of the interview to advise the outcome.

    If you are offered a vacancy, an SDA Service Agreement will be provided to you for completion.

    Should you be offered a vacancy but choose not to accept it, we will respect this choice.

    Where a vacancy cannot be provided, you will be provided with a clear reason based on GR8 Disability Housing’s eligibility criteria, Priority of Access requirements or waiting list processes.

    GR8 Disability Housing may be unable to offer a vacancy where:

    • you do not meet GR8 Property’s SDA eligibility requirements
    • other applicants are assessed as a higher priority based on GR8 Property’s Priority of Access criteria
    • GR8 Property’s SDA does not have the capacity or features to cater to your specific needs; or
    • It is likely that the mix of residents in the available SDA home will be incompatible.

    If you meet GR8 Disability Housing’s eligibility requirements but cannot be offered a service due to lack of capacity, you can be placed on GR8 Disability Housing’s Waiting List. You will be advised of the possible waiting time before a vacancy might become available.

    If you are refused a vacancy you have the right to appeal. Appeals should be directed in writing to GR8 Disability Housing’s Managing Director and a final decision will be made by the team.
